Tender description

Home Care and Support for individuals within the Borough of Darlington. The care and support to be provided is to enable the following; an increase in the number of Individuals being able to continue to live independently their own homes, support Individuals to develop the skills and knowledge they need to live as independently as possible, a reduction in the number of Individuals entering into residential care or nursing home care, a reduction in the number of people who are supported outside of the Darlington area, a reduction in the number of emergency admission to hospital and admissions into Assessment and Treatment Services, the facilitation of timely and safe discharge from hospital and to support unpaid family members and carers to continue in their caring role Lot 1: Standard Support - Zoned Area - Darlington East Home Care and Support Standard Support - Zoned Area - Darlington East Lot 2: Standard Support - Zoned Area - Darlington West Home Care and Support Standard Support - Zoned Area - Darlington West Lot 3: Standard Support throughout the Borough of Darlington (Non-Guaranteed Hours) Home Care and Support Standard Support throughout the Borough of Darlington (Non-Guaranteed Hours) Lot 4: Specialist Support - People with a Learning Disability or Learning Impairment Home Care and Support Specialist Support - People with a Learning Disability or Learning Impairment Lot 5: Specialist Support - People with a Mental Disorder including Mental Health and Dementia Home Care and Support Specialist Support - People with a Mental Disorder including Mental Health and Dementia Lot 6: Specialist Support - People who have Autism including Asperger’s Syndrome Home Care and Support Specialist Support - People who have Autism including Asperger’s Syndrome Lot 7: Standard Support - Support for Families with Children Home Care and Support Standard Support - Support for Families with Children
