Transforming Care - Therapeutic Short Breaks

"This project is focused on the provision of a service for CYP who are experiencing levels of crisis and difficulty which require not only therapeutic interventions but also a facility to provide robust and timely support. Wirral has been an ‘outlier’ in terms of tier 4 admissions for young people with autism, with nine young people having been admitted during the last year . The project aims to build on existing services in providing a therapeutic hub, short break and ‘safe space’ for young people in Wirral. The project will • Provide a facility for up to three young people who are at risk of attendance at A&E, a possible Tier 4 admission, or a placement away from the normal residence. This would provide a short break service in line with the Ealing model adopted and developed on Wirral, offering a range of interventions to support the young person and their carers at a time of crisis and difficulty. • Develop a ‘hub’ across the system (health, social care and education and the third sector) to provide a range of multi service intervention and support.
