<p>Soft Market Questionnaire: Wolverhampton Council is planning to commission in 2021 new services supporting people to access Community Day Activities which can be defined as, &lsquo;providing a meaningful activity and social opportunities which are personalised to meet the needs of the adult.&rsquo; Community Day Activities deliver the following: &bull; Community activities aim to meet the key needs of adults living in the City of Wolverhampton who meet the eligibility criteria and are; &bull; socially isolated; &bull; physically frail &bull; dementia &bull; mental health &bull; Physical or sensory impairment &bull; A flexible service that is responsive to the changing needs and aspirations of individuals; &bull; An ethos of enablement to maximise people&rsquo;s independence; &bull; An outcome-focused model of services; &bull; A continuing commitment to provide carers with breaks from their caring role and assistance to help them continue to care; &bull; A diverse range of provision enabling people to access community facilities and universal services &bull; A Community activity is designed for service users aged 55 and over. For the purpose of the intended procurement activity the focus will be on services for older adults.
