<p>Female client is a 69 year old lady who lives in Peterlee, she has had a recent hospital admission due to Covid 19, presenting with Delirium, confusion , Nausea. Client was out of isolation period on 19-01-21 and has no further symptoms. Client has a medical history of fibromyalgia sciatica, vertebral osteoarthritis, anxiety, fractured radius (2003), dislocated shoulder (2018), vitamin D deficiency, falls, chronic arthritis, osteoporosis and hypertension. Allergies Naproxen and Oramorph. As a result of her ongoing health conditions her mobility has deteriorated &ndash; she now needs a Rota stand and support from 2 people to provide care- Care package required- transfer out of bed /personal care/washing and dressing (30 mins AM 2 Carers) Personal care, hygiene and transfer into bed (30 mins PM 2 carers) Total 14 hours per week.</p> Keywords: home, care
