Tender description

The new Supported Living Framework commenced on August 1st 2021The new Supported Living Framework is inclusive of the provision of personal Support for young people (16+) and adults. This approach will promote effective transition from children's to adult's services and positive outcomes for people as they move into adulthood. It also promotes a more integrated approach to market engagement and development across the Council's services and best use of resources. The Framework is nowcontinually open to new Service Providers to join the Framework at any time throughout the Agreement Term. Lot 1: Lot 1 – Outreach Support Services • Lifelong disability and autism (LLS)• Mental Health (MH)• Live In Care for LLS & MH Care & support provided to a Customer who lives in their own, or a family member’s home. This includes working with Customers towards a reduction of care & support leading to independent living, tenancy sustainability support, and support managing basic everyday living tasks. Lot 2: Lot 2 – Lifelong Disability/Mental Health Accommodation Based Service • Lifelong disability and autism (LLS)• Mental Health (MH)• High need schemes: 24-hour support, 7 days per week, 365 days per year• Medium need schemes: Daily support provided 365 days per year. Night support in place through shared waking night or sleep in.• Low needs schemes: Support is not provided each day. On call arrangements in place for night support. Lot 3: Lot 3 – Intensive and Specialist Accommodation-Based Service • Lifelong disability and autism (LLS)• Mental Health (MH)• High need schemes: 24-hour support, 7 days per week, 365 days per year• Exceptionally specialist schemes for customers with complex needs that require additional staff expertise and an environment to meet needs.
