<p>Sefton Council is seeking an EOI from providers who are able to undertake care and support for a complex individual who is currently placed in a secure hospitalised setting and is soon to be discharged upon stabilisation. Specific Information The individual has various complex needs, predominant needs listed below; &bull; Autism - moderate to severe Learning Disability &bull; ADHD and communication difficulties &bull; Challenging behaviour including; assault on staff/self-harming/self-neglect/ disinhibited behaviours/damage to property / self-injurious Client Requirements &bull; Residential setting &bull; 24 hour Care and Support &bull; Staff team trained in positive behaviour support &bull; Full assistance and support to manage aspects of daily living including recreational activities Service Requirements &bull; Specialist in complex care and support &bull; Innovative approach to providing care and support with proven positive outcomes for clients &bull; Evidence of positive outcomes for clients with similar needs If you think your service can meet the needs and support of this individual with positives outcomes please contact Jacqueline Byrne from the Commissioning Team via email Jacqueline.byrne@sefton.gov.uk , we would be grateful to hear from you.</p> Keywords: Care, Support
