<p>Care package required for a lady who lives alone in a 2 storey house in Romaldkirk, Barnard Castle. She is currently in hospital and ready to be discharge home. Was admitted to hospital with Multifactorial hypoxia and increased O2 requirements, progression in thromboembolic pulmonary hypertension with COVID , acute iron PE, pulmonary Oedema, main driver factor at present. Previous medical history; home oxygen and ( thromboembolic pulmonary hypertension ), essential hypertension, PREV, DVT /PE and iron deficiency. This lady will need 4 x 30 min call per day (AM, Lunch, Tea & PM). Support needed with washing and dressing, bed making and meals and drinks – taking normal diet and fluids. Transfers from bed to chair independently. No issues with continence. No issues with cognition. DOLS; No SALT; No COVID; 1/2/21, has been Covid contact, contact isolation has now ended.
