Supported Living for Complex Health Needs and Severe LD

<p>We are ideally looking for single occupancy accommodation and Supported Living Care for service user M preferably within County Durham, consideration would be given to neighbouring areas. M is in his 20's, has fragile X syndrome and severe Learning Disability, Scoliosis of the spine and severe Epilepsy. In additional M also has communication and sensory needs and can demonstrate behaviours that challenge. These include hitting out, grabbing, kicking, and pulling hair. Personal care &ndash; Needs full support and uses a shower chair, incontinence pads. Night time &ndash; Sleeps well but can be occasionally incontinent at night. Food and Drink &ndash; Totally dependent on others to prepare, cook and shop. SALT guidelines in place with a moist and minced diet with thickened fluids. Sits in a hard rock wheelchair at mealtimes. Behaviours &ndash; Challenging behaviours often triggered when he wants something. If he has to wait, or not understood or asked to do something he does not want to do, hungry or tired, this will trigger challenging behaviours. He will fling himself backwards and therefore wears a helmet, however this is more from a sensory perspective. Keeping safe &ndash; No awareness of risk or danger and needs full support to minimise risk and keep safe. Physical Health &ndash; Epilepsy well managed , only one seizure in 18 months. Prescribed Midazolam. Covid vaccines have been administered. Daily Living &ndash; Fully dependent on others. Communication &ndash; Nonverbal, communicates using gestures, vocalisations, facial expressions and can sign &lsquo;please&rsquo; and &lsquo;yes&rsquo;. Staffing Requirements - 2-1 at all times through the day, including when accessing the community, re-enforced wheelchair as he rocks. 1-1 overnight.
