Rough Sleeper Assertive Outreach and Support Service

<p>To provide a high-quality outcome focused service which reduces the incidence of rough sleeping in the Dorset Council area and establishes a sustainable life away from the streets. The service will do this by: &bull; undertaking regular, street-based outreach work to engage with people who are sleeping rough and ensure they understand the options available to come in; &bull; discouraging activities which may perpetuate a street lifestyle; &bull; encouraging rough sleepers to access housing and support services; &bull; reconnecting rough sleepers who do not have a connection to Dorset Council to return to a local authority they have a connection with; &bull; developing positive and enduring relationships of trust with rough sleepers who have complex needs and/or a history of prolonged or repeat homelessness to find a housing solution and support them in their recovery from first contact until they move into accommodation, ensuring they are equipped to maintain their accommodation and independence and avoid a return to the streets &bull; working proactively with a wide range of services and organisations which can help rough sleepers address their support needs, improve their life skills and, in doing so, achieve positive, sustainable improvements in their housing, health and well-being; and &bull; delivering SWEP ((Severe Weather Emergency Protocol) at short notice; doing evening outreach shifts when this is triggered to ensure all people sleeping rough are offered emergency accommodation for the duration of the severe weather.</p> Keywords: Rough Sleeper, Outreach, Homelessness
