This Notice follows the publication of a Voluntary Ex-Ante Transparency Notice published in Find A Tender on 16th December 2021, reference 2021/S 000-031536. The service is to deliver Continuing Healthcare Services to patients in Nursing Homes commissioned by West Essex Clinical Commissioning Group. The service supports people with long-term complex health needs who have been assessed, and qualify for, NHS funding. The service will be delivered against the local specification which is aligned to the 11 domains of the Decision Support Tool. Lot 1: CHC Nursing Homes - Award to Abbot Care Home The service is to deliver Continuing Healthcare Services to patients in Nursing Homes commissioned by West Essex Clinical Commissioning Group. The service supports people with long-term complex health needs who have been assessed, and qualify for, NHS funding. The service will be delivered against the local specification which is aligned to the 11 domains of the Decision Support Tool. The service has developed the schedules within the NHS formal contract hat, when the contract is issued, will mitigate existing service risks by: - having a specific nursing home specification aligned to the 11 domains of the Decision Support Tool - An agreed rate for both nursing an dementia beds for the contract year - Evidence based requests for one to one support - Provider quality dashboard reporting - Expedites placements, delivering faster acute discharge and improving flow - Enables a strong partnership to be developed with the provider The contract will be for a period of 12 months with the option to extend for a further 12 months. The value for the initial 12 month contract is £2,074,800. If the 12 month extension is utilised this will be £4,149,600. Lot 2: CHC Nursing Homes - Award to Alexandra House The service is to deliver Continuing Healthcare Services to patients in Nursing Homes commissioned by West Essex Clinical Commissioning Group. The service supports people with long-term complex health needs who have been assessed, and qualify for, NHS funding. The service will be delivered against the local specification which is aligned to the 11 domains of the Decision Support Tool. The service has developed the schedules within the NHS formal contract hat, when the contract is issued, will mitigate existing service risks by: - having a specific nursing home specification aligned to the 11 domains of the Decision Support Tool - An agreed rate for both nursing an dementia beds for the contract year - Evidence based requests for one to one support - Provider quality dashboard reporting - Expedites placements, delivering faster acute discharge and improving flow - Enables a strong partnership to be developed with the provider The contract will be for a period of 12 months with the option to extend for a further 12 months. The service supports people with long-term complex health needs who have been assessed, and qualify for, NHS funding. The service will be delivered against the local specification which is aligned to the 11 domains of the Decision Support Tool. The service will be delivered against the local specification which is aligned to the 11 domains of the Decision Support Tool. The service will be delivered against the local specification which is aligned to the 11 domains of the Decision Support Tool.
