K001 The Provision of Falls and Fracture Prevention Services

Tender description

The Falls and Fracture Prevention Service will provide a single point of access for all referrals relating to Knowsley residents over the age of 55 years who are at risk of or have falls. The Provider will deliver: • Provision of a Falls Coordinator role to work across the system to ensure that clear and robust referral pathways are/put in place and utilised e.g. Primary Care, Social Care, Care Homes, Acute settings e.g. A&E, Reablement, Fracture Clinics, Hospital Discharge, NWAS and Pharmacies. • Targeted case-finding e.g. for osteoporosis, frailty, dementia, polypharmacy and falls risk (includes supporting Primary Care to do this). • Delivery of stage two multi-factorial intervention for those at higher risk of falls. • Dedicated care home support ensuring a direct route for falls prevention between service and identified falls lead/champion. • Rapid assessment/intervention and referrals into strength and balance activity community programmes especially for those at low to moderate risk of falls. • Development and Delivery of Training to key partners/agencies, on falls prevention and use of the FRAT and FRAX including, but not exclusive to, care home staff, social care, Dom care, community navigators, fire and rescue, pharmacies and volunteers.
