<p>Jersey has embarked on an ambitious portfolio to transform Health and Social Care on the island through the creation of the Jersey Care Model (JCM) programme.</p> <p>The JCM is based around three core components:</p> <ol> <li>Person-centred care</li> <li>Primary and community care</li> <li>Specialist services</li> </ol> <p>The main objectives of the JCM are to:</p> <ul> <li>Ensure&nbsp;<strong>care is person-centered&nbsp;</strong>with a focus on prevention and self-care, for both physical and mental health.?</li> <li><strong>Reduce dependency on secondary care services&nbsp;</strong>by expanding primary and community services, working closely with all partners, in order to deliver more care in the community and at home.?</li> <li><strong>Redesign health and community services</strong>?so that they are structured to meet the current and future needs of Islanders.</li> </ul> <p><strong>Key Principles</strong></p> <ul> <li>Focus on disease prevention, health improvement&nbsp;and&nbsp;public health&nbsp;&nbsp;</li> <li>Create independence rather than dependence, and promote self-care</li> <li>Moving care into the community&nbsp;</li> <li>Improving Long Term Condition management</li> <li>There is &ldquo;no health without mental health&rdquo;. Mental health is just as important as physical health&nbsp;</li> <li>Reserve the hospital for specialist services&nbsp;</li> <li>Repatriate tertiary services from the UK wherever viable to do so</li> </ul> <p>We are a seeking a supplier, or consortium of suppliers, to build on the prioritisation framework for implementation defined within the JCM, and help us co-design evidence-based, structured, well supported, costed and cost-effective care pathways locally that improve care outcomes for our Islanders</p> <p>&nbsp;</p> Keywords: Care Pathways
