Senior Intervenors for Adults in Long Term Segregation

Tender description

The Authority is looking for Suppliers who can deliver senior consultancy support to resolve challenges and provide oversight to progress the discharge and community support arrangements for a small number of adults who are in long term segregation in mental health hospitals. The primary objectives are to provide intensive case management for between 1 and 5 individual cases including: · To provide support, influence and challenge to senior service leaders (including commissioners and responsible clinicians) · To ensure barriers are overcome for planning and managing the move out of hospital/LTS; and · To support and ensure the needs of people with learning disabilities or autistic people can be met in the community where they have been assessed as ready for discharge (by their responsible clinician and/or multi-disciplinary team). To achieve these suppliers will need to offer persistent overview, challenge and support to the system to enable progress.
