Surrey County Council and Continuing Healthcare (CHC) NHS Surrey (both referred to as the “Commissioner”) are undertaking a joint procurement to identify providers for a Residential and Nursing Care Homes for Older People Dynamic Purchasing System. 'This is a notice for social and specific services in accordance with Directive 2014/24/EU Article 74’ Lot 1: Residential Care with or without Nursing (Older People only) This is for long- and short-term placements (without or without NHS-funded Nursing Care (FNC) funding in place) for people over the age of 65 with needs eligible for social care. Lot 2: CHC Nursing Care (Older People/End of Life fast track only): CHC Nursing Care (Older People/End of Life fast track only): This is for long- and short-term placements for people over the age of 65 with needs eligible for Surrey Continuing Healthcare funding. Lot 3: Discharge to Recover and Assess (Placeholder, Older people only) Discharge to Recover and Assess (Placeholder, Older people only): This will be for the provision of placements with therapeutic input (with or without FNC funding in place) for people over the age of 65 who are being discharged from hospital into a care home for a period up to four weeks. Additional information: Whilst bidders have the opportunity to confirm if they are interested in delivering this service, these lots will not be available at the beginning of this DPS and are currently placeholders for a future requirement which may become available under the DPS. The Council reserve the right to present this lot to bidders in the future, and qualification criteria and detailed specification will be provided then. When this opportunity does become available, all bidders will be made aware via clarification. Lot 4: Residential Care with or without Nursing (Placeholder, Adults for ASC and CHC only 18-65) This is for long- and short-term placements (with or without FNC funding in place – Adult Social Care (ASC) placements only) for people with physical disabilities, sensory impairment (including Deaf / Blind) or a brain injury / neurological condition between the ages of 18 and 65 with needs eligible for either Surrey Continuing Healthcare funding or Adult Social Care. Additional information: Whilst bidders have the opportunity to confirm if they are interested in delivering this service, these lots will not be available at the beginning of this DPS and are currently placeholders for a future requirement which may become available under the DPS. When this opportunity does become available, all bidders will be made aware via clarification Lot 5: Residential Care with or without Nursing (Placeholder, Learning Disability for ASC and CHC only) This is for long- and short-term placements (with or without FNC funding in place – ASC placements only) for people with a learning disability and/or autism (with or without behaviour that challenges) between the ages of 18 and 65 with needs eligible for either Surrey Continuing Healthcare funding or Adult Social Care. Additional information: Whilst bidders have the opportunity to confirm if they are interested in delivering this service, these lots will not be available at the beginning of this DPS and are currently placeholders for a future requirement which may become available under the DPS.
