<p>Brief Description of the Service The Home from Hospital service provides short-term home-based support to individuals being discharged home following a stay in hospital. It aims to support customers to regain their confidence and independence with a combination of both practical (hands on), emotional and social support, following the principle of a planned and phased reduction in support. Alongside this, the service also offers transport home from hospital, if no other alternative is available. The service is also expected to telephone pathway zero patients 48 hours post discharge to ensure that they do not have any outstanding needs. This element requires the signposting of people to appropriate services, both formal and informal.
