An Aylesbury based community centred approach to health and wellbeing

Tender description

Contract awarded following quotation process in line with the council's contract procedure rules. To deliver a health improvement programme in Aylesbury, which will use a range of community centred approaches. From a local presence in the target community the provider will deliver a mixed programme of on-site and outreach Skilled for Health courses and a suite of health improvement activities. To inform the programme the provider will develop, support and mobilise assets within the target communities to engage in the planning cycle, from identifying needs through to implementation and evaluation of the programme. The contract term is 15 months with the option to extend for 12 months.
