Powys Teaching Health Board are seeking to establish a cohort of individuals or companies who are able to act as investigators in relation to clinical, workforce and service issues to support the organisation when it is more appropriate to have external investigators. These matters may include, for example, clinical situations where provision of care and treatment has fallen below expected standards and there is / may be potential harm caused, multifaceted and complex clinical matters spanning more than one health board/ organisation, where an independent clinical investigation out with Powys Teaching Health Board is required, disciplinary allegations, grievance submissions, bullying, sex or racial harassment allegations, whistle blowing/disclosure and any other matter where there is a need to establish the facts or ascertain whether there is a concern.
