AQP (Any Qualified Provider) for Community Optometrist Triage, Assessment and Treatment Service (COTATS) for Lincolnshire CCG

Tender description

The Commissioners require a high quality patient-centred Community Optometry service which raises the standard of care, which is easily accessible and has well-designed pathways for all patients over the age of 16. The service will be available to those patients who are registered permanently or temporarily with a GP within NHS Lincolnshire CCG/ICS. The Commissioners will contract with the Provider. A Performer or number of performers who will then carry out the service under the contract. The Performer/s shall be an optometrist, ophthalmologist or Ophthalmic Medical Practitioner (OMP) and will deliver the following services to patients registered with a General Practitioner (GPs) within Lincolnshire. If the Provider elects to provide services to patients registered outside of Lincolnshire it will be responsible for agreement of appropriate contracts with the relevant commissioners. The Service will diagnose and treat a range of eye conditions within an overall model of care and manage relevant follow-up activity within the service before final discharge back to the care of the patient’s own Optometrist, GP or onward referral to either secondary care or a suitable community based specialist service such as the Community Surgery Scheme.
