<p>Title: Procurement for the provision of Lincolnshire Admission Avoidance (AA), Discharge Support (DS) and Urgent Care Response (UCR) Service to be delivered by a Hospitals Admissions Avoidance Response Team (HART)</p> <p>The aim of this procurement activity is to source suitable, capable, quality provision of health care and support services for patients across Lincolnshire, a county in the East Midlands of England, in the United Kingdom of Great Britain. The service will be procured by United Lincolnshire Hospitals NHS Trust (ULHT) Lincolnshire Procurement Service on behalf of and for the lead commissioner and contracting authority: Lincolnshire Community Health Services (LCHS); working in collaboration with Lincolnshire County Council (LCC), and in partnership across Integrated Care System (ICS) health and social care partner organisations.&nbsp;</p> <p>United Lincolnshire Hospitals NHS Trust is issuing this Invitation to Tender in connection with the competitive procurement of Lincolnshire Admission Avoidance (AA), Discharge Support (DS) and Urgent Care Response (UCR) Service to be delivered by a Hospitals Admissions Avoidance Response Team (HART). Lincolnshire Community Health Services NHS Trust is &ldquo;the Contracting Authority&rdquo; to this Tender and resulting contract. Lincolnshire County Council are an additional named user of the service. United Lincolnshire Hospitals NHS Trust are acting as the agent of the Contracting Authority in all matters relating to this procurement. &nbsp;</p> <p>This service contract is offered with a maximum prospective budget of &pound;1,134,516.00 per annum. This service contract is for a maximum potential duration of 5 years, 3 years with an option to extend for 2 years further, and the commercial schedule has been set out accordingly planning for this duration. Bids that do not deliver within and therefore exceed the budget will be disqualified from the process.&nbsp;</p> <p>The service and contract will commence on the 01 April 2022 to 31 March 2025 for the term of 3 years with an option to extend for up to 2 years (12 or 24 months). The total maximum opportunity will therefore be up to five years with a maximum end date of 31 March 2027. The maximum prospective value of contact terms is therefore &pound;5,672,580.00 over 5 years.&nbsp;</p> <p>This is an open invitation to tender (ITT) procurement process.
