NHS Sheffield CCG - Physical Health Outcomes - SMI High level outcomes: - National SMI annual physical health check targets are improved and met in Sheffield. - GP practices will be able to report an increase in the number of SMI annual physical health checks completed and report a positive experience of the additional support provided by the service. - Covid, flu and pneumonia vaccination rates for people with SMI will increase.
