ENQ738 - Provision of Habilitation Services

Tender description

Derby City Council is seeking to appoint a Service Provider to provide a suitably qualified and experienced Habilitation Specialist (HS) to assess the orientation, mobility and appropriate independence needs of children and young people with a visual impairment, whose residential address is within the Derby City Council boundary. The aim of service is to meet the orientation, mobility and appropriate independence needs of children and young people (Service Users) with a visual impairment. The agreed total maximum funding over the total contract period, including any extensions, is £72,000. The maximum budget per year is £18,000. Any bids that are calculated to be over this value per year will be disqualified. There is potentially an additional £18,000 available over the total contract period subject to further funding being agreed but this is not guaranteed. Bidders must meet the minimum requirements of the main suitability questionnaire as detailed in Section 4.3 of the ITQ Part One - Process Overview Document.
