<p>POC required for a 92-year-old gentleman who lives with his son, who is his main carer, in a privately owned two storey farmhouse in rural location. He is due to be discharged home on the 29-JAN-21 without care, as he unable to remain in hospital until care can be sourced and son wants him home and happy to support as he was doing prior to admission. He requires a package consisting of 2x carers 3x daily, or even 1x carer if happy to work with the son as second carer, to meet his day to day needs and to support his son in his caring role and prevent carer breakdown. This care package is very important for the well-being of this gentleman and his son, who has been the sole carer for many years, due to no care agencies being able to pick up the package as a result of where they live. Physical Health – Past medical history of CVA left sided weakness, left leg amputation, Chronic Kidney Disease, Hypertension, long-term catheter Mental Health – No behaviour or management issues. No issues with mental capacity, confusion or disorientation. Gentleman is able to make his own decisions and choices. No concerns raised regarding mental health and well-being. Personal care – He is needing support from carers with personal care and clothing tasks. He is nursed in bed and has limited mobility due to a previous stroke and left leg amputation. He requires assistance from carers to monitor, wash and cream pressure areas daily to prevent skin deterioration. Hospital bed and mattress is in place. Toileting needs – Gentleman has a long-term catheter in place which he will need assistance to change bags as needed. Gentleman has bowl incontinent and will need regular checks and assistance with continence needs and changing pads, as required. Medication – Son supports with medication. Diet and nutrition – Son supports with all meals and drinks. Package of care required – 2x carers, 3x daily calls or 1x carer to work with son as second carer.
