Tender description

The aim of the service is: to provide an innovative person-centred prevention and support service to individuals at risk of, or who have a diagnosed stroke or hospital admission for a TIA. The client base is varied and will include vulnerable people and people with complex needs including co-morbidities. to provide a quality, person-centred service that offers choice and flexibility to meet service user and carers needs. to provide a family-centred service that ensures those caring for Stroke / TIA survivors have the knowledge, skills, and attitudes to provide safe, compassionate, and effective care. to provide timely, responsive support to help the service user and their family rebuild their life and manage the changes they need to make as a result of stroke; ensuring they are able to cope long-term with the physical, social and psychological effects and live a full life in their home, in their community, preventing further strokes.
