Somerset County Council and the NHS Clinical Commissioning Group are currently in the Discovery stage of a project called Help to Help Yourself. The project vision is to ensure that people have clear and easy access to Information and support in Somerset when and how they need it in order to live healthy and positive lives. If you are interested in providing a response please express an interest via the Supplying the Southwest portal.
