Community Based Wellbeing Specialist Dementia Services (Lots 7-12)

Tender description

Together, these six contract Lots provide support for people living with dementia and their families across Kent and a Dementia Coordinator Service across Kent and Medway: Lot 7 – Post Diagnostic Support in the Community for People with Dementia and Their Families in West Kent Lot 8 – Post Diagnostic Support in the Community for People with Dementia and Their Families in East Kent Lot 9 – Post Diagnostic Support in the Community for People with Dementia and Their Families in North Kent Lot 10 – Dementia Coordinator Service in West Kent Lot 11 – Dementia Coordinator Service in East Kent Lot 12 - Dementia Coordinator Service in North Kent (including Medway) The Contracts awarded for Lots 7, 8 and 9 will run for a period of 3 years (expiring 31st March 2025), with the option for two extensions of up to one year each at the discretion of the Council. The Contracts awarded for Lots 10, 11 and 12 are have been procured by KCC on behalf of Kent & Medway CCG and will run for a period of 2 years (expiring 31st March 2024).
