Mid & South Essex STP is an innovation-focused health and social care network with the leadership, governance architecture and stakeholder partnerships in place to provide fertile ground for innovators, entrepreneurs and partners across industries to create, test and spread novel ideas and solutions in the real world.Through creating this Dynamic Purchasing Framework, we seek partners who can work with us in health and social care to achieve this ambition, at pace and with an improvement approach which tests, learns and adapts over time to be changing for the better, and as our experience and the evidence base related to better value and outcomes grows. Lot 1: Lot 1 - Innovation Partnership Innovative partners with novel products relevant to Mid and South Essex FT and local system strategic priorities, where a project is identified and agreed as suitable for the Mid and South Essex Health and Care Partnership. Current priorities include reducing demand for face to face outpatient care, improving patent empowerment and self-care, workforce experience and digital alternatives to acute care. Lot 2: Lot 2b - digital triage Partnership with clinical and digital services at MSE FT to develop a shared solution for digital triage involving patient-derived content, supporting underlying scoring algorithms and direct integration with both primary and secondary care EHR platforms Lot 3: Lot 2c - digital patient pre-consultation Design and delivery of a system which supports patient pre-consultation questionnaires and pre-consultation diagnostics. Lot 4: Lot 2a - digital referral and advice Partnership with clinical and digital services at MSE FT to develop an ‘all digital’ advice and guidance referral pathway, compatible with ways of working locally, using asynchronous messaging and “phone a consultant’ options Lot 5: Lot 2d - infrastructure, in/out patient monitoring Working with MSE to build infrastructure to support patient initiated follow up and clinician remote patient monitoring, including creation of personalised health and care plans for all patients and sign-posting to condition specific health literature / videos and clearly articulated warning indicators to help patients self-manage conditions and identify when they need to seek clinical input. Lot 6: lot 2e - patient access Partnership with clinical and digital services at MSE FT to develop a shared solution for patient access to their clinical record, directly integrated patient self-service for appointments, digital letter delivery and, where relevant, connected device and asynchronous messaging support for patients.
