Bed Based Discharge to Assess is a service that provides an opportunity for Adults and Older People to have a short term stay in a residential or nursing home setting following a period in hospital. The term 'Bed Based' is used to describe the environment in which the service will be delivered. This service is to be used to provide • a period of ongoing support ,assessment and review to ascertain a persons progress towards recovery and independence • a supported discharge from hospital option for people who are medically fit to be discharged but who require the opportunity to undergo a period of supported recovery and assessment to ensure the most appropriate care and support services can be put in place to meet their needs and aid their return home where possible. • an opportunity for people to make decisions about their future in an unpressurised environment and consider the most appropriate next steps in meeting their needs
