Tender description

The Penderi Cluster is made up of 6 general medical practices (Fforestfach Medical Centre x 2 practices, Manselton Surgery, Cheriton Medical Centre, Brynhyfryd Medical Centre and Cwmfelin Medical Centre), 4 Dental practices, 3 Opticians 9 Community Pharmacies, 2 Care Homes, 6 Community Centre’s and 12 schools (11 Primary and 1 Secondary). It works with partners from key local authority departments such as social services and poverty and prevention as well as the voluntary sector and wider Swansea Bay University Health Board. The Penderi Cluster aims to continue the good work undertaken within the area for supporting young people aged 9 to 17 years with their emotional wellbeing. The project also extends support to a young person’s family/carers should their needs be identified as having an impact on the young person or if they require guidance on managing their child’s needs. The PYPWP is effectively a social prescribing plus model sitting between the GP surgery and CAMHS offering young people and their families a suite of support options: - A full needs assessment of both the young person and their family/carers from a trauma informed perspective; - Up to four 121 sessions for the young person; - Up to four 121 sessions for each of the young person’s family members/carers; - Motivational interviewing techniques, psycho-education and social prescribing; - Funding of therapeutic interventions sourced from a variety of providers (3rd Sector and private), based on the identified needs of the young person or their family/carers; Budget for reducing barriers to engagement for families (childcare, transport etc…) NB: This Tender Opportunity is for the Penderi Cluster but the project has the potential to be adopted by other Cluster areas. Appointing a Young Persons Mental Health Worker will allow unmet needs to be identified and be the central point of referrals within the Penderi Cluster. Key Facts: • 140 individuals have been supported by the project in the past 18 months (2019-2020) • 66% of referrals required support for a parent/carer or sibling (which highlighted the need to work systemically). • 100% of the young people assessed experienced at least one adverse childhood experience (ACE), with many experiencing 4 or 5 significant events in their lives. These traumas are largely uncovered through support and are often inconsistent with the presenting needs highlighted by the GP.
