Tender description
This contract is to support Wirral’s NHS led Discharge to Assess (D2A) service for assessment of people for their ongoing needs for those who do not require acute NHS care but are unable to return/stay in their usual place of residence. This will provide timely discharges and community step up to ensure system flow is achieved
