Tender description

The Homeless Out of Hospital Care Model is a test and learn project funded by the Department of Health and Social Care through the Shared Outcomes Fund. Qualitative and quantitative data will be collected and analysed to inform a national evaluation and the future homeless hospital avoidance and discharge pathway. The model will be delivered in Central Cornwall from January 2021 to March 2022. This will include intensive care and support for typically six to twelve weeks through the following: A. Hospital network Homeless Patient Advisor roles B. Temporary Accommodation units with onsite support C. Intermediate Community Outreach support with daily living tasks D. Intermediate Personal Care delivered as in-reach and outreach
