<p>This is an opportunity for the provision of NHS Funded Nursing Care (including Continuing Healthcare) in Care Homes in Northamptonshire, under the Any Qualified Provider (AQP) process.</p> <p>Successful applicants will be required to work collaboratively with the CCG to provide high quality care to patients registered with a Northamptonshire GP who have been assessed as meeting the eligibility criteria for NHS continuing healthcare, NHS Funded Nursing Care or who are on a D2A pathway.
