Through the Your Life, Your Way programme, The aim of the East Riding Local Coordination model is to offer a person-centred approach, focused on prevention and co-production, to help individuals lead lives with greater control and independence. This approach moves away from traditional approaches to providing support in a time-generous, patient centred, empowering and flexible way. Local Coordinators will take the time to get to know individuals and families, build understanding of their strengths and their current challenges, and listen to stories of what has works well for them in the past and what hasn't, supporting them to develop their own plans for improving their wellbeing and take action as a result.
