Mental Health Rehabilitation Services (2021-22)

Tender description

This service provides a time limited mental health rehabilitation service in a supported housing setting for high needs clients with mental health problems. It also provides support to four residents with lower level mental health needs in another property. All referrals to this service seek the approval of Adult Social Care prior to being referred and often come from hospital or high needs residential settings to assist with step down into the community. This service prevents the blocking of hospital beds by responding to discharge plans.
