Tender description

The Call-Off Contracts will seek to fulfil the requirement to ensure timely hospital discharge and transition via the ‘Hospital to Home’ service as well as delivering Services within the community to prevent hospital admission. These services are of particular criticality in the current Covid-19 pandemic situation, and its effects during the winter.
