Public Health Tier 2 Weight Management Service

Tender description

Service to support eligible residents of Hampshire to lose weight. Lot 1: The service will provide effective, evidence-based, and future-proofed interventions to enable adults to achieve meaningful and sustainable weight loss and maintenance of this weight loss, in order to benefit their current and future health. The population covered by this service are resident in the administrative county of Hampshire (or registered with a Hampshire GP) and meet the acceptance criteria. The service will be paid on performance and the total value stated is up to £2,290,000.00. The service should focus on but not be limited to; f. People living in the three most deprived Hampshire deciles of deprivation g. Men h. People diagnosed with long-term conditions i. Black, Asian, and Minority Ethnic (BAME) groups j. People with learning disabilities The acceptance /inclusion criteria for this service are as follows: • Population aged 18+ years with a BMI ≥ 30 (BMI ≥ 28 black/minority ethnic origin or those with specified long-term conditions), resident in the administrative county of Hampshire or Registered with a Hampshire GP • Young people aged 16 and 17 years (with GP consent) The service provides both a face-to-face setting-based approach and a digital approach.
