Tender description

The Inverse Care Law programme aims to reduce the inequality in healthy life expectancy that is experienced by people living in the most deprived areas in North Wales and increase years of healthy life. This aim will be realised by achieving the following objectives within the programme: • Identify people who live in the most deprived areas in north Wales (most deprived and second most deprived fifths); • Engage with the community in order to outreach people and encourage them to attend a health and wellbeing check with a Health Care Support Worker (HCSW), within their local community (either in a GP practice or an appropriate community venue); • Undertake a health*and wellbeing check*; this will include undertaking a risk assessment of the individual’s risk for cardiovascular disease and type 2 diabetes; • Undertake a Mental Health risk assessment such as the Warwick Edinburgh Scale questionnaires • Discuss (based on motivational interview techniques) of any identified risks and the relevant clinical, risk factors and risk conditions exploring community-based interventions that can reduce/ eliminate the identified risks; for instance Social Prescribing and Community Assets. • Improve individual health management and enhance people capacity and responsibility of taking care of themselves. This will result of enabled community efficacy, literacy and self-care. • Onward referral using evidence-based pathways of care, to relevant clinical or community-based interventions, dependent on the risks identified and the priorities of the individual; • Collation of data on outcomes from health and wellbeing check, including the impact of the interventions received on their quality of life and wellbeing. *Health and wellbeing check to include: blood pressure, heart rate and rhythm, lipid profile, HbA1c if at high risk of type 2 diabetes, smoking status, BMI, Mental Health **Wellbeing check to discuss wider determinants of health including housing and financial concerns
