Out and About Service - Sensory Support

Dumfries and Galloway Council on behalf of the Dumfries and Galloway Health and Social Care Partnership are seeking a community based facilitation service for people living with sensory impairment that will operate across the region. The Service will provide a service that is personalised, offers flexibility and choice, is delivered in a way that empowers people to take informed decisions, treats people with dignity and respect, and is provided in a way that respects people’s privacy. Services to be delivered will include: -Specialist advice, information, assessment and facilitation; -Handles personal information in confidence; -One to one and group facilitation to enable people with a sensory impairment to identify and work towards outcomes to enhance and improve social inclusion; -Working with people with sensory impairment in identifying existing and new opportunities within local communities and signposting for users to specialist resources, such as employment, education, life skills training and leisure; -Maintain and add to the database that is currently in place, which signposts user onto other specialist resources, such as information on educational courses available at Dumfries and Galloway College -Working with a range of other agencies and individuals to develop activities in relation to individual aspirations, for example, finding or changing employment; -Working with others in a multi-agency approach to deliver appropriate therapies or treatments, for example Dumfries and Galloway Council and the Health and Social Care Partnership; -The service will not include providing personal care on a regular and planned basis; -Costs – cost of accessing activities and events will be met by the person using the service. The provider will meet any such cost incurred by its own staff or volunteers when facilitating person / people to access activities. Fundraising – the provider will undertake appropriate fundraising to raise funds to facilitate the development of activities and to alleviate person / people those who may experience hardship/difficult in meeting the cost of activities.
