Stroke Recovery Service on behalf of SWLCCG - Wandsworth Borough

Tender description

This service provides support and information to those who have had a stroke and their family and/or carers and then offers an opportunity for a health and needs review 4-8 months following the stroke. This is to discuss their recovery and rehabilitation progress and offer further opportunities for functional improvement and wider support. The preferred provider will work with patients and their family/carers to identify individuals' post stroke unmet needs across health, social and emotional care domains. The review must cover the following areas: • Medicines, secondary prevention strategies, general health needs • Mood, memory, cognitive and psychological status • On-going therapy and rehabilitation needs • Social care needs, benefits and finance, driving, and transport • Return to work/vocational needs • Needs of the carer(s)
