Tender description

The purpose of the tender is to develop a framework for service providers which will then be used to call off the delivery of a Complex Needs Pathway. For individuals stepping down from a secure hospital or stepping up from a community setting. Short/medium term transitional placement that does not require a nurse led service. 24-hour, higher end supported living with access to a specialist MDT. 24-hour core service core of cluster model.
