The In lieu of short-term enablement service is for adults who have been assessed as requiring a period of intermediate care whilst determining their longer term support needs. The service is designed to provide a high quality locality based intermediate care service. The Service will provide flexible, intensive, consistent and integrated short term support to enable an adult to remain at home or return to their normal place of residence. The service will provide support to adults to maximise their independence and achieve their optimum level of function, usually in their own home (but support could also be delivered within other settings including a relative’s home, a residential setting or temporary accommodation), including a rapid response when required. This could be to enable an early and safe transfer from hospital, to prevent admission to hospital, or to meet an immediate support requirement within the community.
