Grants to provide rehabilitation, equipment, support, information and advice for adults with a hearing impairment

Grant applications are invited from Service Providers with relevant experience and ability to demonstrate sufficient capacity to provide a provision of rehabilitation, equipment, support, information and advice for adults with a hearing impairment. Grant applications are welcome for all or part delivery of the services noted below. Aim of the service: To achieve the best possible outcomes and quality of life for adults living in Somerset with a hearing impairment, in order for them to maintain or improve their independence and to prevent, delay or reduce individual need for more costly care and support. It is anticipated this will be achieved via the delivery of: 1. Information and advice 2. Signposting to local and National support 3. Access to specialist resources, including equipment 4. Lip- reading classes Objectives of the service: • To provide a sensitive, quality, person-centred service that offers choice and flexibility to meet individual needs. • To support adults with a hearing impairment to maximise their independence, social, emotional and physical well-being. • To empower adults with a hearing impairment to make choices by informing them of their rights and the support available to them. • To support the development of services for people with a hearing impairment in Somerset via involvement with Somerset Community Connect Development Group • To develop knowledge of and build effective working partnerships with key agencies, organisations, community and voluntary groups • To raise awareness of hearing impairment and promote the independence and well-being of people with a hearing impairment including: personal dignity and control over daily life; physical and mental health; social and emotional well-being; protection from abuse or neglect; participation in work, education, training, and recreation; economic well-being; and suitable living accommodation. o To work co-productively recognising the key characteristics of co-production: o Recognising people as assets o Building on people’s existing capabilities o Promoting mutuality and reciprocity o Developing peer support networks o Breaking down barriers between professionals and recipients o Facilitating rather than delivering Outcomes: The Commissioners want to take a proactive and innovative approach to results from local providers. In showing how the service is achieving its aim the provider will develop, to the satisfaction of people with a hearing impairment and Commissioners, outcome measures to support and compliment the performance and effectiveness of the service being provided. The outcomes being sought for people with a visual impairment include: • Having improved quality of life, • Having increased confidence in their abilities and motivation to achieve goals, • Being better able to adjust positively to changed circumstances and cope with the changes they need to make to their life as a result of reduced hearing, • Having reduced anxiety and worry, knowing that knowledgeable people are there to listen, support and guide them, • Having increased awareness and knowledge of their rights and support available, • Being able to lead safer and more independent lives making the best use of the hearing they have, • Being better able to access facilities and support in their community, • Being better able to find, regain or remain in work and access other occupational and educational opportunities.
