City of Bradford Metropolitan District Council (‘The Council’) is seeking to commission a service which: Provides an alternative to respite in a care home for people with identified social care needs and their carers using personal budgets support for people and their carers who can self fund to plan for holidays. The provider will identify, source, arrange and make bookings on behalf of disabled people, unpaid carers and the people they support to access a wide range and mixture of free, discounted and full cost breaks and socialising activities. The provider will identify, source, arrange and make bookings on behalf of disabled people, unpaid carers and the people they support to access a wide range and mixture of free, discounted and full cost breaks and socialising activities. The service will: — provide up to date information (face to face, telephone, paper based and web based) on opportunities and options for breaks and activities, — provide a range of opportunities and options for breaks and activities that are appropriate to the diverse communities within the District, — provide opportunities for individuals to specify their requirements, — provide a personalised and researched response to enquiries based on the individuals specific requirements, — undertake home visits where necessary and requested and where the individual is unable to make use of the telephone or written information and cannot access the service centre, — where appropriate, communicate with the providers of brokered opportunities on behalf of people with identified social care needs, unpaid carers and the people they support to facilitate uptake of any opportunities brokered and arranged. — undertake on-going publicity, promotion and awareness raising activities about the service with the private, commercial, voluntary and charitable sector, — seek opportunities, in partnership with other groups and organisations where appropriate, — secure additional funding/resources to support the service including funding to support people with identified social care needs, unpaid carers and the people they support, — gather feedback on user experience of the service and provide opportunities for people using the service to contribute to the development of the service as it adapts to meet emerging needs and trends, — utilise web-based resources, including Bradford’s Connect to Support facility to gather and provide information on potential opportunities available to people using the service, — demonstrate innovative ways of designing, promoting and delivering services from the outset.
