Tender description

Community based non-clinical support to people experiencing mental health crisis and ongoing severe and enduring mental illness. Provision of 6-day per week services to: — maintain and improve recovery, — build self-confidence, — improve physical well-being, — improve social networks including educational attainment, — reduce dependence on mental health services. Counselling service to: — enhance quality of life and health gain, — enhanced perception and use of GPs, — improve appropriateness of referrals to linked services and agencies. Helpline and information service to optimise service access. Piloting of support to improve outcomes for A&E attenders experiencing mental health crisis with focus on proactive care in collaboration with clinical partners. Provision of community-based services for patients within Croydon. These are non-clinical voluntary sector led services delivering to people experiencing a mental health crisis and ongoing severe and enduring mental illness. These services support achievement of the NHS outcomes framework and the following locally defined outcomes for social networking 6 days/week: (a) maintain and improve on their recovery; (b) build self-confidence; (c) improve physical well-being; (d) improve social networks beyond mental health services; (e) improve personal skills, including educational attainment; (f) reduce dependence on mental health services, where appropriate. The counselling service will achieve the following outcomes. For the client — enhanced quality of life and health gain. For the primary health care team — enhanced perception by GPs and other team members that their time and skills are utilised more appropriately and an increase in care options offered and more appropriate care provided. For local service commissioning and providing agencies — more appropriate referral patterns and usage of services and effective usage of resources.
