Tender description

North Tyneside CCG are interested in speaking to providers in the market place with relation to the provision of their community integrated frailty service. The service will improve care for the frail elderly residents of North Tyneside by offering them rapid, equitable, access to a range of specialist health and care interventions in order to prevent admission and/or facilitate discharge from hospital. The service will include: — a single point of access that will be responsible for accepting referrals, carrying out an initial triage and deploying resources accordingly, — an urgent response function that will respond to acute deterioration, providing early interventions to avoid conveyance to A&E/admission to hospital, — a planned response function that will provide multidisciplinary assessment, care planning and interventions in order to prevent further deterioration and enable better self-management of frailty, — access to 40 intermediate care beds on a step-up/step-down basis. Also required is the provision of space to accommodate other teams, associated with wider service delivery.
