Tender description

East Lothian Council, on behalf of East Lothian Health and Social Care Partnership (ELHSCP) is seeking expressions of interest from organisations who would be capable of delivering a Community Link Worker service in East Lothian. The Community Link Worker service in East Lothian will deliver a social prescribing model which is aimed at tackling social isolation and health inequalities for patients registered with East Lothian GP Practices. The social prescribing model is likely to be based around the use of structured conversations with service users, the provision of basic level advice and supporting individuals to engage with local community resources. The service would also be expected to engage with local community groups to promote the development of suitable activities and groups to meet patient needs. Where possible the Community Link Workers will be based in East Lothian GP practices and will also undertake wider engagement with the health and social care partnership and local community services.
