NHS NEL CSU on behalf of NHS England London Region invites suitably qualified and experienced providers to deliver APMS Services for a number of GP practices in London. The practices are: Lot 1: Bursted Wood Surgery (DA7), Bexley; Lot 2: Queens Road Surgery (SE15) Southwark; Lot 3: Silverlock Medical Centre (SE16) Southwark; Lot 4: Cator Medical Centre (BR3) Bromley; Lot 5: Camden; Lot 6: Brunswick Medical Centre UHPC (WC1N) Camden; Lot 6: Brunswick Medical Centre UHPC (WC1N) Camden; Lot 7: Ordnance Unity Centre for Health (EN3) Enfield; Lot 8: Newham Transitional Practice (E12) Newham; Lot 9: Allerton Road Medical Centre (N16) City and Hackney; Lot 10: Carpenters Practice (E15) Newham; Lot 11: Goodman's Field (E1) Tower Hamlets; Lot 12: Abbotts Park Health Centre (E10) Waltham Forest; Lot 13: Waterside Medical Centre (UB2) Ealing; Lot 14: Heston Great West Practice (TW4) Hounslow; Lot 15: Earls Court Health and Well-being Centre (SW5) West London. Please note all the contract values for the lots are stated for the maximum 15 year term. The authority is seeking to appoint a provider of primary medical care services for the patient list registered with Bursted Wood Surgery. The practice is currently managed under an APMS contract. Bursted Wood Surgery is located in the London Borough of Bexley and it is part of Bexley Clinical Commissioning Group (CCG). The current contract for the provision of primary medical care services is due to expire on 31.12.2019 and hence the need to commence this procurement. The authority is seeking to appoint a provider of primary medical care services for the patient list registered with Queens Road Surgery. The practice is currently managed under an APMS contract. Queens Road Surgery is located in the London Borough of Southwark and NHS Southwark Clinical Commissioning Group (CCG) is the local delegated commissioner. The current contract for the provision of primary medical care services is due to expire on 31.12.2019 and hence the need to commence this procurement. The authority is seeking to appoint a provider of primary medical care services for the patient list registered with Silverlock Medical Centre. The practice is currently managed under an APMS contract. Silverlock Medical Centre is located in the London Borough of Southwark and it is part of NHS Southwark Clinical Commissioning Group (CCG). The current contract for the provision of primary medical care services is due to expire on 31.12.2019 and hence the need to commence this procurement. The authority is seeking to appoint a provider of primary medical care services for the patient list registered with Cator Medical Centre. The practice is currently managed under an APMS contract. Cator Medical Centre is located in the London Borough of Bromley and it is part of Bromley Clinical Commissioning Group (CCG). The current contract for the provision of primary medical care services is due to expire on 31.12.2019 and hence the need to commence this procurement. The practice is currently managed under an APMS contract. CHIP is located in the London Borough of Camden and it is part of Camden Clinical Commissioning Group (CCG). The authority is seeking to appoint a provider of primary medical care services for the patient list registered with Brunswick Park Medical Practice. The practice is currently managed under an APMS contract. Brunswick Medical Centre is located in the London Borough of Camden and it is part of NHS Camden Clinical Commissioning Group (CCG). The current contract for the provision of primary medical care services is due to expire on 31.3.2020 and hence the need to commence this procurement. The authority is seeking to appoint a provider of primary medical care services for the patient list registered with Ordnance Unity Centre for Health, 645 Hertford Road, Enfield, Middlesex EN3 6ND. The practice is currently managed under APMS contract, and will be under a caretaking arrangement until 31.3.2020. Ordnance Unity Centre for Health is located in the London Borough of Enfield and it is part of NHS Enfield Clinical Commissioning Group (CCG). The current contract for the provision of primary medical care services is due to expire on 30.9.2019 and hence the need to commence this Procurement. The contract will be in caretaking from 1 October to 31.3.2020, whilst the procurement process is being undertaken and concluded. The authority is seeking to appoint a provider of primary medical care services for the patient list registered with Newham Transitional Practice, Vicarage Lane Health Centre, 10 Vicarage Lane, Stratford, E15 4ES (main site) and Church Road, 30 Church Road, Manor Park, E12 6AQ (branch site). The practice is currently managed under an APMS contract. Newham Transitional Team is located in the London Borough of Newham and it is part of NHS Newham Clinical Commissioning Group (CCG). The current contract for the provision of primary medical care services is due to expire on 31.3.2020 and hence the need to commence this procurement. The Authority is seeking to appoint a provider of primary medical care services for the patient list registered with Allerton Road Surgery, 34A Allerton Road, London, N16 5UF. The practice is currently managed under an APMS contract. The Allerton Road Surgery is located in the London Borough of Hackney and it is part of City and Hackney Clinical Commissioning Group (CCG). The current contract for the provision of primary medical care services is due to expire on 31.3.2020 and hence the need to commence this procurement. The authority is seeking to appoint a provider of primary medical care services for the patient list registered with Lantern Health (Carpenters Road), 236-252 High Street, Stratford, London, E15 2JA which is the main site. There are 2 other branch sites: — Lantern Health (St Luke’s Practice), 2 St Luke’s Square, Canning Town, London, E16 1HT, — Lantern Health (Church Road Practice). The Centre Manor Park, 30 Church Road, Manor Park, London, E12 6AQ. The practice is currently managed under an APMS contract. Lantern Health (Carpenters Road) is located in the London Borough of Newham and it is part of NHS Newham Clinical Commissioning Group (CCG). The current contract for the provision of primary medical care services is due to expire on 31.3.2020 and hence the need to commence this procurement. The Authority is seeking to appoint a provider of primary medical care services for the patient list registered with East One Health, 14 Deancross Street, London E1 2QA and its branch premises at Cable Street Surgery, 445 Cable Street, E1W 3DE. The practice is currently managed under an APMS contract. It is planned to relocate the practice from its current locations in Deancross Street and Cable Street to a new purpose-built premises 0.8 miles from Deancross Street at Goodman’s Fields, within the first year of the contract i.e. by August 2021. NB Whilst it is planned that the practice relocates to Goodman’s Fields within the first year of the contract, this will be subject to developer construction timescales and may be subject to a contract variation. It is further planned that another existing neighbouring Tower Hamlets practice, Whitechapel Health Centre, will also move into the new premises at Goodman’s Fields at some point prior to the natural expiry of its APMS contract (30th September 2024) and the two practices will be expected to share reception and back office functions whilst operating as two individual practices. At the natural expiry of Whitechapel Health Centre’s contract, it is planned that Whitechapel will merge with East One Health from 1.10.2024 to operate under the single APMS Contract that is being procured in this procurement. The APMS contract in this procurement will contain contractual and financial provisions for the planned future relocation and merger. East One Health is currently located in the London Borough of Tower Hamlets and it is part of Tower Hamlets Clinical Commissioning Group (CCG). Both the proposed new site and Whitechapel Health Centre, with whom it is planned East One Health subsequently merges, are located within Tower Hamlets CCG and borough. The current contract for the provision of primary medical care services at East One Health is due to expire on 8.8.2020 and hence the need to commence this Procurement for an APMS contract commencing 9.8.2020. The authority is seeking to appoint a provider of primary medical care services for the patient list registered with SMA Medical Centre, 693-695 High Road, London, E10 6RA. The practice is currently managed under a caretaking APMS contract. SMA Medical Centre is located in the London Borough of Waltham Forest and it is part of Waltham Forest Clinical Commissioning Group (CCG). The current contract for the provision of primary medical care services is due to expire on 31.3.2020 and hence the need to commence this procurement. The authority is seeking to appoint a provider of primary medical care services for the patient list registered with Waterside Medical Centre. The practice is currently managed under an APMS contract. Waterside Medical Centre is located in the London Borough of Ealing and it is part of Ealing Clinical Commissioning Group (CCG). The current contract for the provision of primary medical care services is due to expire on 31.3.2020 and hence the need to commence this procurement. The authority is seeking to appoint a provider of primary medical care services for the patient list registered with Heston Great West Practice. Heston Great West Practice is located in the London Borough of Hounslow and it is part of Hounslow Clinical Commissioning Group (CCG). The practice is currently managed under a caretaking APMS contract. The current caretaking contract for the provision of primary medical care services is due to expire on 31.3.2020 and hence the need to commence this procurement. The authority is seeking to appoint a provider of primary medical care services for the patient list registered with Earls Court Health and Wellbeing Centre. The practice is currently managed under an APMS contract. Earls Court Health and Wellbeing Centre is located in the Royal Borough of Kensington and Chelsea and it is part of West London Clinical Commissioning Group (CCG). The current contract for the provision of primary medical care services is due to expire on 31.3.2020 hence the need to commence this procurement.
