Tender description

The provision of a service for adults aged 18 and over with unpaid caring responsibilities, caring for adults aged 18 and over, who are eligible for adult care services in the county of Suffolk and patients who are the responsibility of the Suffolk Clinical Commissioning Groups (CCGs), including Ipswich and East Suffolk Clinical Commissioning Group (IESCCG), West Suffolk Clinical Commissioning Group (WSCCG) and the Waveney part of Great Yarmouth and Waveney Clinical Commissioning Group (GYAWCCG) areas (referred to as the CCGs). The service will include the following four specific areas: 1) assessment and review; 2) support; 3) information, advice; and 4) guidance and identification of carers.
