<p>What we are doing? Sheffield City Council (along with NHS partners) are exploring the potential of developing a Rapid Re-ablement Dementia Service. At this stage there is no guarantee that it will be taken forward but we are interested in your thoughts about whether there is capacity and capability in the market to deliver something of this nature. Why are we doing this? We have identified this as a potential gap in supporting people living with dementia who are either leaving hospital/care home and/or who need rapid access to support at home to prevent an unnecessary admission to hospital or a care home.</p> Keywords: dementia
