Cambridgeshire County Council is extending block contract arrangements with a number of Care Homes to provide interim beds. These provide short-term bed based care to people who are leaving hospital but not yet ready to return home, or who need additional support during a period of illness as an alternative to hospital admission. These provide short-term bed based care to people who are leaving hospital but not yet ready to return home, or who need additional support during a period of illness as an alternative to hospital admission. The extension is needed to maintain provision whilst a transformed care pathway is commissioned to support hospital discharge. The new care pathway will support an increased number of older people to return home after hospital discharge, reducing demand for long term bed-based care. It is anticipated the new care pathway will comprise of a range of short stay beds for step up and step down purposes, along with respite and reablement interventions and in-reach domiciliary care.
