Carers Support Services (Adults and Children or Young People)

Southampton City Council (the ‘council’) in partnership with Southampton City CCG (the ‘CCG’) is seeking to appoint service provider(s) to deliver two contracts. The first is for a range of universal support services for adult carers and the second for targeted support services for young carers. Both services will be available to all carers living in Southampton and will be equipped to respond to the substantial increase in the numbers and varied demands of carers. Both will also be expected to work closely with health and social care providers and community organisations to ensure a 'joined-up' holistic approach in identifying and meeting carers support needs. The service for adults will: • be delivered in a manner that reflects the City’s diverse population to include, (but not restricted to) older carers, BME and transition groups; • work collaboratively to sustain established local carers support groups and current providers of high standard carer’s services to build on existing good practice and enhance service provision where relevant • be proactive in the early identification of new carers, providing support, advice/information and signposting in a variety of formats, available during the day, evenings and week-ends; • recognise the individual needs of some carers and will tailor services to meet their requirements as far as possible; • provide link-workers (including family link workers) to work with organisations including primary care and hard to reach groups to deliver effective support to carers and families; • identify and support high priority carers; carers who may shortly be bereaved, have an increase in their caring responsibility, under significant pressure, from a diverse background, in a co-dependent role or older carers to minimise feelings of isolation, desperation or breakdown of caring role; and
